Please make checks payable to GIST CANCER RESEARCH FUND. All contributions are tax-deductible and will be used for GIST research. Tax ID # 13-4182988-501(c) (3) status. The consultant and medical advisor for the Fund is Dr. Ephraim Casper from Memorial Sloan Kettering Cancer Center.
Please bring this form and all contributions with you on the day of the Walk. If unable to attend, please forward contributions to GCRF, 55 Saw Mill Road, New City, NY 10956. Many of the doctors that perform research and care for GIST patients will be present for the NY Walk.
